Tips to Prepare for Your Doctor Visit

At Hematology and Oncology Consultants of Pennsylvania, we are not only focused on providing you with the very best care – we are also focused on making your visits with us as easy for you as possible.

You can help by bringing the following to every appointment:

  1. Driver's license. If you don’t have a driver’s license, please bring another state or federal form of photo identification. A scan of your ID will be added to your chart during your first visit.
  2. Insurance card. Your insurance card is needed for our office to verify your health insurance information for billing. If you have a pharmacy benefit card, please bring that with you as well.
  3. List of current medications. It is important for your doctor to be aware of any prescription or over-the-counter medication you are taking and the doses. Bring a list or bring the bottles themselves.
  4. Patient Health History Form. For your first appointment, fill out this form to the best of your ability and bring it with you. The form is part of the New Patient Packet we send to all new patients.
  5. List of questions. Write down all the questions you have for your doctor prior to your visit. This will help make sure that all your questions are answered and all your concerns are addressed.
  6. Friend or family member. If possible, bring a family member or trusted friend with you to your visit. It will be helpful to have that individual with you to be a second set of ears.

Now that you know what to bring to your visit, here is something to take off your list: stress. We aim to make scheduling appointments and visiting us easy and stress-free.
